In the article, basic pathogenic mechanisms of progression ofcerebral atherosclerosis leading to ischemic stroke are discussed.The ultrasound, morphological and biochemical predictorsof changes of the atherosclerotic plaque structure arepresented. A multifactorial nature of pathogenesis of progressingcerebral atherosclerosis as a key cause of ischemic strokedetermines the need for further studies of different aspects ofthis problem, from vessel and brain morphology to metabolicparameters and various blood characteristics (biochemical,lipid, conformational, and hemostatic).
