Simultaneous occurrence of subfrontal extradural hematoma with periorbital hematoma following head injury is a very rare event. We report a 12-year-old-male who fell from a running train and sustained faciocranial injury. Glasgow coma scale was 13/15 on arrival and he had left sided proptosis. CT brain has revealed subfrontal extradural hematoma with orbital subperiosteal hematoma, which was also associated with orbital roof fracture. Patient had made good clinical recovery following left frontal craniotomy and evacuation of the subfrontal and subperiosteal hematoma. We stress the importance of orbital CT scan whenever patient present with the subfrontal extradural hematoma with the orbital roof fracture to rule out orbital subperiosteal hematoma.
