Mesenteric lymphatic malformation (MLM) is a rare disease. Most cases of symptomatic MLMs present with abdominal pain and distension and may have localized abdominal masses. The standard treatment is complete resection. However, in diffuse-type MLMs, complete resection has a high risk of complications, such as short bowel syndrome. Alternative treatment options for macrocystic MLM include sclerotherapy and fenestration. Conventionally, localized macrocystic MLMs are easily resectable or only require alternative treatments. Herein, we report a rare case of diffuse microcystic MLM with volvulus, which had a high risk of developing complications following complete resection. Conservative management or alternative treatment options are recommended for such high-risk cases.
