Cerebrospinal fluid leak (CSF) due to granulomatosis with polyangiitis (GPA) is a rarely described complication of sinonasal GPA. We report the case of an 80-year-old man found to have a spontaneous CSF leak arising in the context of severe sinonasal GPA. Surgical repair of the cribriform plate defect was performed with no evidence of recurrence at last follow up. Our findings illustrate the importance of early diagnosis and treatment of GPA and highlight some surgical nuances associated with repair of spontaneous CSF leaks in the context of GPA.
