Abstract This Critical Synthesis Package contains: (1) a Critical Analysis of the psychometric properties and application to health sciences education for the Hospital Survey on Patient Safety Culture (HSOPS), and (2) a copy of the HSOPS instrument in English and in Spanish developed by the Agency for Healthcare Research and Quality (AHRQ), as well as the 2014 comparative report that can be used as a resource for organizations as they analyze their outcomes. The HSOPS measure was developed by the US government organization AHRQ in 2004 to assess the current safety environment and provide potential foci for improvement. The instrument includes 42 questions and measures responses on a 5-point Likert scales (1 = strongly disagree, 5 = strongly agree). Eight demographic questions are also included to identify the respondent's role and event-report data. An open-ended question on patient safety, error, or event reporting is also included. An overall grade is then provided. Since its inception, HSOPS has become the most frequently used tool for assessing patient safety culture. It has been used by an extremely large volume of respondents both in the US and internationally, and has undergone repeated analysis by the developers and others. HSOPS results have shown repeated reliability and have been positively correlated with decreased adverse patient events (predictive validity). Outcomes also correlate well with the Safety Attitudes Questionnaire (SAQ), another validated patient safety culture survey.
