Design und prototypische Implementierung eines IT-Lösungskonzepts für eine Versorgungsmanagement-Plattform mit Integration der EFA

Abstract

The aim of this research work was to implement a prototype of an electronic application with integration of the EFA to support IT-based care management processes. A solution concept has been designed to describe an implementation of a care management scenario consisting of the EFA and a care management platform. This work is as research approach as part of the increasing efforts of the federal government to promote the use of electronic application in order to achieve an improvement in the quality of patient care. Through the representation of defined patterns of interaction and the integrated functional model of the EFA, the care management platform is able to support a central process control, communication and data exchange in collaborative care management. The solution concept is the basis for the implementation and can serve as reference for similar systems.
