Here is presented a case of a 22-year-old female with an extensive cervical giant cell bone tumor comprising about 80% of the oropharynx and laryngopharynx. A combined posterior and transmandibular approach was needed to accomplish a complete resection and stabilization. The post-operative recovery was successful, and the patient experienced notable improvements in neurological function, pain relief, and cervical spine stability. This case highlights the effectiveness of this uncommon approach in treating upper cervical lesions.
