Abstract This report describes a patient who presented with a large thyroid nodule and compressive symptoms. Immunohistochemical staining showed diffuse marked reactivity for vimentin and focal reactivity for CD68 and Ki‐67 that is compatible with primary undifferentiated pleomorphic sarcoma of the thyroid.
