Organization of transport of patients from primary health care institutions to secondary and tertiary level institutions

INTODUCTION: Transport involves the transfer of patients from primary health care to the health care institutions of secondary and tertiary levels. Transport is carried out in an ambulance accompanied by a doctor and / or nurse. AIM: To describe a way of organizing the transport of patients within the emergency medical services (EMS) Health Center (DZ) Becej. MATERIALS AND METHODS: We used a retrospective observational study that was conducted in the emergency department DZ Becej. The target group were patients who, during the period from 01.01.2013. to 31.12.2013. was cared for and transported to a medical institution of higher levels in the company of doctors and / or nurses. Data were collected from the outpatient protocol, the protocol field teams and transport protocol drivers. RESULTS: During the 2013 EMS DZ Becej transported 969 patients. EMS physicians have sent 495 (51.08%) patients, while the remaining of 474 (48.92%) have send by the doctors of other departments DZ Becej. From the total number of patients transported, accompanied by a doctor-nurse was sent to 123 (12.69%) patients, and 303 (31.27%) only accompanied by a nurse. The remaining 543 (56.04%) patients were transported accompanied by relatives. The nearest hospital is 45 km away from DZ Becej, which means that despite the good will, good organization and teamwork, transport time interval is 35 minutes. CONCLUSION: One-third of patients in the secondary and tertiary centers was transported accompanied by nurses. Although the most of these patients were not critically ill, during the transport we can expected worsening of their health. The role of nurses in transport is to provide good comfort to the patient and gain his trust, but primarily to monitor the vital parameters of the patient. Of an adequately responding of nurses during the transport depends patient's life in case of deterioration.
