Summary:. Tie-over bolster fixation is the most common method of securing a skin graft. The bolster material can consist of various materials, including sterilized polyurethane foam. Mepilex Ag is a silver-containing polyurethane-foam wound-dressing material that has a broad-spectrum antibiotic effect. The use of a silver-containing polyurethane foam as bolster material in tie-over bolster fixation has not been reported previously. We report here our method of a multilayered silver-containing polyurethane-foam tie-over bolster fixation and its outcomes when used during reconstruction of a relatively small skin defect on the nasal dorsum with a full-thickness skin graft. The skin graft was completely viable, and there were no complications such as infection, hematoma, or necrosis.
