Arterial heart disease has been increasing sharply in the past few years, and cardiac surgery is one of its forms of treatment. Surgery is an overwhelming and unique situation in patients’ and their families’ lives. Thus, we have decided to develop a practice aiming at promoting process-involved health trying to re-educate their habits. To achieve this goal, McGill’s Nursing Model and Carraro’s Care Model were used. In this article, we have presented how this practice was carried out; it was proposed in the discipline of Nursing and Health Assistance Projects from the Postgraduation Program of Federal University of Santa Catarina.
