A 28-year-old single female diagnosed with transverse sinus thrombosis; she was also discovered to be diabetic with severe hyperlipidemia. She presented by recurrent episodes of diffuse dull aching abdominal pain referred to the back; she was diagnosed with acute pancreatitis with markedly elevated triglyceride (TG) levels. She started medical treatment for hypertriglyceridemia with no improvement. She started four sessions of plasma exchanges with marked improvement of the general condition, abdominal pain and appetite, and TGs declined. While the patient was on plasma exchange she developed catheter related infection so it is stopped and soon triglycerides raised again with recurrence of the abdominal pain and we started insulin infusion with improvement of her condition
