Vertebrobasilar dolichoectasia in CADASIL: A new aspect that needs consideration

Abstract

Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is an autosomal dominant disorder of the brain microvasculature, which results in formation of multiple small areas of ischemia and demyelination in the white matter. Presence of large vessel abnormalities in patients with CADASIL is very unusual. To our knowledge, our patient is the first reported case of concomitant Vertebrobasilar dolichoectasia and basilar tip aneurysm in CADASIL, presented with acute infarction in posterior circulation territory. In cases of infarctions in unusual locations for CADASIL patients (such as posterior fossa), we recommend evaluating large vessel abnormalities with computed tomography (CT) or Magnetic resonance (MR) angiography in order to prevent further complications.
