Introduction: A broken scalpel blade during surgery is not often unheard of, what is more, its legal implication of retained fragmented pieces in the body. The retrieval procedure is challenging, tedious and increases morbidity. Discussion: We illustrate a case of broken surgical blade during lumbar discectomy. The blade was retrieved with the help of fluoroscopic guide and advanced microscope. A two-surgeons technique and choice of surgical instruments are described. Noteworthy surgical tips to a safe procedure are shared. Conclusion: Maximal usage of the correct equipment and meticulous approach is the key to fishing the broken blade out in a single setting surgery.
