Introduction: Isolated tibiotalar dislocation is an extremely rare lesion. It is due to high velocity mechanisms of injury. Case presentation: The authors report the case of an acute closed isolated posterior tibiotalar dislocation in a 28-year-old patient following a road traffic accident. The reduction was performed as an emergency procedure under general anesthesia followed by a 6-week cast immobilization without weight bearing. Conclusion: It is rare for a tibiotalar joint dislocations to occur as an isolated injury especially in its posterior form. Our observation illustrates the place of urgent orthopedic treatment in the management of this lesion.
