A 20-year-old male patient diagnosed with a complex iatrogenic ureteral lesion in a single right kidney underwent diversion of the urinary tract using a ureterostomy. He is referred to the Urology service of the Hospital General de México with acute kidney injury and pain in the right flank to preserve renal function. Medical treatment is given and exploratory laparotomy is performed due to the total absence of the right ureter, complete ureteral replacement is performed with cecal appendix, during clinical evolution multidisciplinary management is given. The importance of this case is to present a treatment option in complex ureteral lesions, ureteral replacement with cecal appendage, which is currently an unusual technique, however with multiple advantages. This technique is a suitable option in the total absence of the ureter to preserve the renal function of the patient’s only kidney.
