Congenital high airways obstruction syndrome (CHAOS) is a rare condition consisting of malformations of the larynx (diaphragm, stenosis, atresia) or trachea possibly associated with other organ congenital anomalies. Diagnosis can be established by ultrasound and MRI starting from the first trimester of pregnancy. Disease incidence cannot be assessed, many of the affected children dying at birth or in utero. The authors discuss the management of such a patient, comparing interventional fetoscopy and emergency post birth tracheostomy as alternatives, in cases where CHAOS does not associate other organ malformations.
