INFERIOR LIP SPINOCELLULAR CARCINOMA ETIOLOGIC FACTORS-NOVEL ETIOLOGIC ASPECTS CASE REPORT

Introduction Squamous cell carcinoma of the lower lip represents 90% of all oral cancers and its incidence is increasing. Material and methods Our study focuses on two cases of patients with carcinoma of the lower lip, both men: first case, an 68 years old man, with chronic alcohol consumption, smoker (2 packs / day over 30 years), with partial edentation, from rural area, under chronic intermittent Miofilin treatment for COPD underwent a wide excision in the lower lip for ulcerative squamous cell carcinoma confirmed histologically. Risk factors in this case are: male, age over 50 years, chronic smoking, alcohol consumption, prolonged sun exposure, lack of dental care. The second case, a man of 42 years, urban area, with occasionally consumption of alcohol, ex-smoker, with no comorbidities, is diagnosed with squamous cell carcinoma of the lower lip, infiltrative form, confirmed histologically after excision of large tumor. HPV (Human Papilloma Virus) DNA was detected in the cells obtained by the scraper of the oral mucosa through PCR amplification techniques. Etipatogenic factors in this case are: males, HPV infection. Results and conclusions A careful review of the factors involved in the etiopathogenesis of the lower lip squamous cell carcinoma include: exposure to ultraviolet rays in the elderly (intermaxillary distance is reduced gradually due to dentition changes causing vermillion's protrusion of the lower lip and thus favoring prolonged contact with carcinogens – UV rays, cigarette/cigar); chronic alcohol consumption; smoking at a young age; poor oral hygiene; partial or temporary edentation; Helicobacter pylori - recently isolated from dental plaque, the dorsal tongue and the inner lip; HPV infection; xeroderma pigmentosum
