Mammary-type myofibroblastoma is a rare benign neoplasm of mesenchymal origin. Presentation outside of the embryological mammary crest is exceedingly rare. This case report documents the presentation of a 40 year old male who presented with a painless infra-auricular swelling. We discuss histological, clinical and radiological findings as well as the challenges of managing spindle cell lesions within the head and neck region.
