We report a case of primary tubulovillous adenocarcinoma of the upper urinary tract in a 51-year-old man who presented with pyuria, mucosuria, and recurrent stone formation. The pathogenesis is postulated to be associated with chronic irritation, infection, inflammation, hydronephrosis, and urinary calculi. This case report highlights the pathological characteristics and management of this rare neoplasm.
