Acro metastasis is an unusual occurrence, typically linked to lung malignancies. This is a case report involving a 50-year-old woman who sought medical attention at the outpatient department due to persistent leg pain along the outer side. Radiological assessments revealed an expansive lesion in the neck of the fibula with a well-defined border and soft tissue swelling. Further staging investigations identified a lesion in the lingular region of the right lung. A core needle biopsy of this lesion confirmed the presence of lung adenocarcinoma. Immunohistochemistry staining confirmed the diagnosis by testing positive for NAPSIN A and TTF1. This case represents a rare report of adenocarcinoma of the lung metastasizing to the fibula.
