Abstract Background Type IIIb endoleak is a rare complication after endovascular aneurysm repair (EVAR) and the understanding of type IIIb endoleak is insufficient. Case presentation Three elderly patients with previously successfully performed EVARs were sent to our center due to abdominal pain and diagnosed with late type IIIb endoleak. The type IIIb endoleak fabric tears were closed with cuffs or stents by endovascular retreatment. The patients recovered adequately and were discharged from hospital. Conclusions Endovascular repair with empirical relining could be a good choice for treatment of late type IIIb endoleak with good prognosis.
