We report an original case of a 17-year-old girl with osteochondral fracture following traumatic patellar dislocation of the left knee. What makes this case rare is the unusual site of the osteochondral detachment: the posterior portion of the lateral femoral condyle (pCFL). Since this site is difficult to access, a combined arthroscopic and open approach was preferred to reduce and synthetize the osteochondral fragment with resorbable pins. After 6 months, the patient appears asymptomatic, with good clinical and functional outcomes and a progressive return to sports activity.
