Abstract Generalised pustular psoriasis (GPP) is characterised by a widespread rash with pustules and fever. Clinical and the histological overlap with the delayed hypersensitivity reaction acute generalised exanthematous pustulosis may delay the diagnostics and the initiation of correct treatment. In this case‐report, we describe an Human immunodeficiency virus positive patient with GPP possible triggered by monkeypox vaccination and eventually successfully treated with spesolimab, an interleukin‐36 inhibitor.
