Prostatic Leiomyosarcoma is a rare tumor with less than 200 cases reported. These are aggressive tumors and treatment guidelines are not well established. In early stages, they usually present with obstructive lower urinary tract symptoms without elevation of serum prostate-specific antigen. They lack specific clinical or imaging characteristics, hence keeping a high degree of skepticism might aid in timely detection. The early-stage disease can be managed with surgery and radiotherapy with relatively modest survival. Here we are reporting a case of primary prostatic leiomyosarcoma with some insight into diagnostic issues and disease biology with basic management principles. We have developed a treatment algorithm based on available literature.
